Sensory Motor Stage Piaget’s first stage of development is the sensory motor stage. This stage occurs between the birth of the child and the age of two. During this stage, understanding comes from touching, sucking, chewing, and manipulating objects. About nine months after birth, the child develops what is called ‘object permanence’. Object permanence is the awareness that objects and people continue to exist even if they are out of sight. The infants have the ability to build up mental pictures of objects around them, from the knowledge that they have developed on what can be done with the object. Through manipulation, babies accumulate information on themselves and the world that lead to the slight understanding of how one thing can cause or affect another, and begins to develop simple ideas about time and space. An example of this would be that a baby can realize that if they cry when they are hungry, the mother will attend to them (Fleck, 1975, p. 3). Preoperational Stage Piaget’s second stage of development was the preoperational stage. The preoperational stage of development occurs between the ages of two to seven years. During this stage, children’s though processes are developing. There is a development of language and use of symbols. Children still use egocentric thought, meaning that they view the world entirely from his or her own perspective. ‘Animism’ is also a characteristic of the preoperational stage. This is when a person has the belief that everything that exists has some kind of consciousness. An example of this would be that a child would believe the sink isn’t turning on because it is sick or that the water will be hot because it’s angry. A child at this stage of development appears to view his social relationships and the physical reality egocentrically. This means that they view the world with a marked tendency to evaluate interaction with others in terms of its contribution to their own experience of satisfaction. So moral realism is an aspect of this stage because children think that their thoughts on the difference between right and wrong are shared by everyone else around them. (Appel, 1977, p. 4). Concrete Operational Stage Piaget’s third stage of development is the concrete operational stage. The  concrete operational stage of development occurs in children between the ages of seven and twelve. Before the beginning of this stage, children’s ideas about different objects are formed and dominated by their appearance. An example of this is that they believe there are less toys when they are all piled up rather than spread out across the floor because it takes up more space on the ground. During this stage, the thought process becomes more rational, mature, adult-like, and operational. Children in this stage of development lose their egocentric frame of thought and begin to think logically. This especially is true for the child’s ability to develop logical thought about an object that they are able to physically manipulate. These children have difficulty understanding abstract, hypothetical questions. Children at the concrete-operational level would be expected to draw on the experie nces of others in evaluating their environment, giving more realistic and natural. (Koocher, 1973, p. 2). Formal Operations Stage Piaget’s last stage of development is the formal operational stage. The formal operational stage of development begins at the age of around eleven or twelve and is fully achieved by the age of fifteen and taken throughout the rest of adulthood. The structures of development become the more abstract, logically organized system of adult intelligence. There are two major characteristics of formal operational thought including ‘hypothetic-deductive reasoning’ and ‘propositional reasoning’. Hypothetic-deductive reasoning means that when faced with a problem, the person is able to come up with a general summary of all the possible factors that might affect the outcome, and the different outcomes possible. Propositional reasoning means that adolescents can focus on verbal assertions and evaluate their logical validity without making reference to real-world circumstances. In concrete operational development, children can only evaluate the logic of statements based off of concrete evidence. Formal operational development brings critical, theoretical, and problem-solving types of thought that gives them much more thought and understanding than they had in the past. (Koocher, 1973, p. 8). â€Å"Are we forming children who are only capable of learning what is already known? Or should we try to develop creative and innovative minds, capable of  discovery from the preschool age on, throughout life?† –Jean Piaget On August 9, 1896, developmental psychologist and philosopher Jean Piaget was born. Jean was the first psychologist to make a systematic study of cognitive development. His contributions include a theory of cognitive child development, detailed observational studies of cognition in children, and a series of simple but ingenious tests to reveal different cognitive abilities. Before Piaget’s work, the common assumption in psychology was that children are merely less competent thinkers than adults which he disproved showing the strikingly different ways children think in comparison to adults. Piaget’s theories of child development continue to be studied in the field of education. His theory differs from others in several ways. For one, it is concerned with children, rather than all learners. It also focuses on development rather than learning so it does not address learning of information or specific behaviors. It proposes discrete stages of development marked by qualitative differences, rather than a gradual increase in number and complexity of behaviors, concepts, and ideas.

A. Assessment of Patient The assessment of Mrs. Baker should include vital signs including pulse oximetry. Given her difficulty in breathing, lung sounds should be auscultated. Because she is on two different medications that could affect blood pressure, lisinopril and hydrochlorothiazide (HCTZ), hypotension could be one cause of her collapsing. In addition to vital signs, decreased peripheral pulses and capillary refill can also be indicators of hypotension. Also, because HCTZ is a diuretic, dehydration should be considered. Since she was in her backyard when she collapsed, it could be that she was doing yard work. Thus, it is important to know what she was doing before she collapsed. The outside temperature and whether Mrs. Baker became overheated or drank enough fluids could be useful factors in assessing her fluid-electrolyte balance. Serum electrolyte levels and vital signs should be taken, and color and skin turgor should be assessed. Dehydration could also contribute to hypotension. Mrs. Baker should be placed on a cardiac monitor and an EKG obtained to rule out any dysrhythmias. Given Mrs. Baker’s respiratory symptoms and her history of hypertension and diabetes (which are the two biggest risk factors for congestive heart failure), a chest x-ray should be done. Arterial blood gases could assist in further assessing her oxygenation, as well as acid-base balance. Also, since Mrs. Baker is a diabetic, a blood glucose level should be done, since hypoglycemia could be a cause of her mental status changes. When Mrs. Baker is alert, pain can be assessed through a verbal â€Å"numbers† scale, or a â€Å"faces† scale if she is mildly confused. When she becomes unresponsive, non-verbal indicators of discomfort should be observed, such as muscle tension, grimace, and grunting. B. Technological Tools A stethoscope is an essential assessment tool. It is needed to assess breath sounds, heart sounds, and perhaps assist in taking the blood pressure. A stethoscope works by amplifying internal sounds through a diaphragm and transmitting them through a hollow tube to the earpieces. (Schunk, p. 1) The sounds can be interpreted by a skilled assessor and used to make a diagnosis and prescribe treatment. In Mrs. Baker’s case, it would be extremely important to listen to her lung sounds to determine if there are changes associated with atelectasis, or more likely, to assess for fluid sounds in the lungs, which would be an indicator of congestive heart failure. If a manual blood pressure measurement is being taken, the stethoscope would also be essential to listening for restored peripheral blood flow as the pressure in the cuff is being released. The benefit of a stethoscope is that it is a simple tool that can be used to easily obtain an initial assessment of a patient’s breath sounds while waiting on more complicated assessments, such as blood gases and chest x-rays. If a manual blood pressure cuff is not available, blood pressure will probably be obtained using an electronic blood pressure device such as a Dinamap. A blood pressure measurement, as well as other vital signs, would be key in determining whether Mrs. Baker was suffering from hypotension related to her new medication, lisinopril. A manual blood pressure cuff has the benefit of being more accurate at extremely high or extremely low pressures, while an electronic device, such as a Dinamap, can be set to automatically check blood pressure at regular intervals. Many cardiac monitors also have blood pressure capabilities built in, so that may be an option for obtaining blood pressure. A cardiac monitor would be used for Mrs. Baker to watch for arrhythmias, which could be caused by lisinopril, or be another sign of congestive heart failure or an acute myocardial infarction. Mrs. Baker has a history of hypertension and diabetes, which are the two greatest risk factors for heart disease. An electrocardiogram (EKG) would be useful for diagnosing heart disease or damage as well. A cardiac monitor is essential in continuous monitoring of the patient, but a 12 lead EKG has the benefit of being able to better pinpoint the specific areas of the heart that are affected or damaged. Blood sugar can be obtained from a drop of blood with a glucometer such as One Touch, or it may be obtained along with the serum chemistry and electrolyte analyses from the lab. A glucometer is a quick and easy way of determining the blood glucose level. Mrs. Baker is a diabetic, and is on an antidiabetic drug, Metformin. Her change in mental status could be due to low blood sugar. This would be easily diagnosed with a glucometer, and easily treated with IV glucose, so it should be one of the first assessments made. A glucometer has the benefit of being a quicker and easier way to obtain a blood sugar level, while a serum glucose may be more accurate at extremely high or low levels. Pulse oximetry can be done with a dedicated pulse oximeter, or it may also be built in to the capabilities of the cardiac monitor. Since Mrs. Baker is suffering from respiratory difficulty, it is essential to find out her blood oxygen saturation level. An arterial blood gas would not only be helpful in giving a more precise measurement of arterial oxygen and carbon dioxide, it would also give useful information about the pH of her blood and the bicarbonate level. Pulse oximetry has the benefit of alerting caregivers to acute changes in blood oxygenation level, while arterial blood gases gives a more detailed picture of the of the state of gas exchange in the lungs Serum electrolytes would be measured by the laboratory. These would be helpful in diagnosing dehydration and/or kidney function. Electrolytes are also essential in cardiac function. In fact, serum electrolytes are a good overall way to assess homeostasis in a patient. A radiographer will use an X-ray machine to obtain the chest x-ray, either in the radiology department, or will use a portable model. A chest x-ray is another way to assess respiratory function. A chest x-ray could show fluid in the lungs, as well as atelectasis or infiltrate. It would also show cardiomegaly, which is another indicator of congestive heart failure. A portable chest x-ray would be beneficial, as it would not require Mrs. Baker to be taken from the emergency department. Finally, if Mrs. Baker has been a patient at this facility before, the Electronic Medical Record can make it easy to find information on her past medical history. This is an improvement over medical records of years past, which usually had to be located in extensive file cabinets or even on microfiche. Sometimes these records were lost or misfiled. Electronic Medical Records are usually easy to locate. C. Data Collection Prioritization Vital signs, pulse oximetry, and a visual assessment should always be the first assessments performed on a patient. The severity of the vital signs will determine whether emergency life-support interventions need to be initiated at once, and will give the caregivers clues to what needs to be looked at next. Since hypoglycemia is an emergency situation, a finger-stick blood sugar should be the next assessment formed. And since Mrs. Baker is having respiratory difficulty, breath sounds should be assessed as soon as possible to determine what measures need to be taken to insure adequate ventilation. Lab work, x-rays, and other tests should be done after the initial assessments, since the results will take longer to obtain. D. Pain Assessment Comparison In alert and oriented patients with no cognitive impairments, the best way to measure pain is to use the â€Å"numbers† scale. Using this scale, you have the patient rate their pain on a scale from 0 to 10, with 0 being no pain, and 10 being the worst pain they can imagine. Pain is subjective, meaning that it can only be judged by the person in pain, and not by an observing party. The numbers scale gives an easy way for a person to tell her nurse the level of her pain, and to judge how much it is relieved by interventions. A geriatric patient who is alert but has mild cognitive impairments may be confused by numbers, but can be given the Wong Baker faces pain scale to help to measure their pain in the same way as the numbers scale. A patient who is not alert or who is unconscious is obviously not going to be able to describe her pain to her caregiver. In this case, the caregiver will have to observe the patient for non-verbal signs of discomfort. These include grimace, muscle tension, increase heart rate, respirations, or blood pressure, and moaning or grunting. E. Pain Management If Mrs. Baker was not alert enough to respond to questions but was showing signs of pain, and given the standing orders of acetaminophen or morphine, I would elect to give the patient morphine. The patient is not alert enough to take any medication by mouth, and an oral medication will take longer to work. Intravenous Morphine works very quickly, and may decrease the work of breathing in patients with congestive heart failure. The caregiver should reassess the patient’s pain after giving any pain medications. In the case of a non-alert patient, the caregiver would look for signs such as stabilization of vital signs and muscle relaxation. I have learned that the geriatric patient has a decrease in physiologic reserve, and therefore there is a shorter time between changes in homeostasis and a state of shock. Because of this, when an elderly patient is showing signs of distress, assessment and interventions should be done quickly to improve outcome. I also learned that an elderly patient may live their everyday life with a certain level of chronic pain, so when they complain of acute pain, it should be taken seriously and treated promptly. F. Team Members Mrs. Elli Baker may have been assessed initially by an EMT or paramedic, if emergency medical services had been called to transport her to the emergency room. Upon arrival, she would have been assessed by a nurse, and at the same time or shortly thereafter, the emergency room physician. The nurse would have obtained vital signs, applied the cardiac monitor, and obtained the finger stick blood sugar, while the physician would have given an initial set of orders. A respiratory therapist would have probably been called, given her respiratory difficulties, and an EKG technician would have been called to do an electrocardiogram. Labs may have been drawn by a phlebotomist or a nurse, and the arterial blood gases would have been obtained by the respiratory therapist or a phlebotomist, depending on the facility’s policy. Finally, a radiology technician would have performed the chest x-ray. When the nurse noticed that Mrs. Baker became unresponsive and started having more difficulty breathing, she would have notified the physician of the changes and called any necessary support staff to assist with Mrs. Baker’s immediate care.

Paul is more frail than his brother was, and he is very artistic rather than athletic. As Paul grows in sight of his mother her love for him grows stronger because of his stature, and he grows in a way that he cares of what other people feel, especially his mother. He develops an Oedipus Complex towards his mother. â€Å"Oedipus complex makes mother a lover of her son. During childhood it seems normal and acceptable but it is abnormal and almost absurd, when he grows up, to keep such feelings. Gertrude sees this and feels threatened by her because Pauls soul should belong to her, she believes Miriam can have him physical but not spiritually. Gertrude confronts her son about this matter and Paul tells her that he does not love Miriam, that he loves only his mother and no one else. When Paul breaks things of with Miriam and starts a relationship with Clara, Gertrude is content with them together because Clara is only interested in a physical relationship. When the relationship with Clara fades Miriam tries her luck with Paul again, she says that she wants to marry him but Paul is so fixated with his love for his mother that when he feels the slightest feelings for Miriam he feels as though he is betraying her. He declines the proposal from Miriam and goes back to his mother. Gertrude Morel was a strong woman and mother but stubborn with her children in a way that she would want them to only look to her and not to walter, no matter what the situation was. Throughout the novel Gertrude ruins three lives, that of her her husbands, and her two eldest sons. Her husband Walter was not the best man for her but he did try to be a father to his children, but his wife made it almost impossible for him to do so. Post Operative Care After Gall Stone Removal Bianca Robinson Patients who are undergoing operative procedures are required the delivery of ongoing care to optimize their recovery and prevent complications. This delivery of care will enable early identification of circumstances surrounding surgery that may put patients at risk of harm. Mr Whakanna is a 36 year old Polynesian male who has just returned to the ward after having a laparoscopic cholecystectomy. A laparoscopic cholecystectomy is the surgical removal of the gall bladder using laparoscopic technology in a process also known as keyhole surgery (Graham, 2008, p. 47). The aim of this report is to identify and prioritize the problems associated with in the first four hours of Mr Whakaana’s return. It is important for nurses to have an understanding of gallstone disease and the surgical procedure, to ensure that patients are cared for with empathy but also safely and effectively. This report presents the four highest problems that may occur with Mr Whakaana on return to the ward fro m surgery. ABCD’s, Vital Signs and Pain Although different surgical procedures require specific and specialist nursing care, the principles of post-operative care remain the same. It is essential for a structured assessment of Mr Whakaana to be carried out such as that described by Elliot, Aitken Chaboyer (2007) where Airway, Breathing, Circulation, Disability and Environment are examined. This is known as a primary assessment, and is used to identify any signs of airway obstruction, respiratory failure, circulatory failure or neurological dysfunction (Graham, 2008). In this scenario, the nurse must pay particular attention to Mr Whakaana’s airway due to the fact that he has been administered 8mg of morphine, and morphine can cause respiratory depression (Tiziani, 2010). Bradypnoea is a respiratory rate less than 12 breathes per minute in an adult at rest, and is the first sign of respiratory depression; Mr Whakaana should be monitored closely to prevent this (Tiziani, 2010). Mr Whakaana’s conscious state sh ould also be monitored especially as he is currently scored as 1 on the Glasgow Coma Scale, the nurse must pay particular attention to this to ensure that Mr Whakaana does not go into shock (Elliot, Aitken Chaboyer, 2007). It is also helpful to include the patency of drainage systems and vascular devices into your primary assessment of Mr Whakaana, and note if any allergies are known (Elliot, Aitken Chaboyer, 2007). Vital signs should be assessed as often as possible (every half hour/hour) during the first four hours of Mr Whakaana’s return to the ward to determine any signs of deterioration. Vital sign measurements include blood pressure, respirations, pulse, temperature and oxygen saturation levels. Changes in Mr Whakaana’s blood pressure can be used to monitor changes in his cardiac output; pulse assessment can determine Mr Whakaana’s heart rate and rhythm, and can estimate the volume of blood being pumped by his heart (Elliot, Aitken Chaboyer, 2007). Core body temperature differences can occur in illnesses and an abnormal reading can be an indication of infection; Mr Whakaana’s temperature is 36.5C at present, which is within normal range (REFERENCE). Pulse oximeters give a non-invasive estimate of the arterial haemoglobin oxygen saturation, and measurement should always be above 95% (REFERENCE). The nurse should be aware that Mr Whakaana is currently on 3L per m inute of oxygen via nasal prongs, as this could give a false sense of security when recording/documenting Mr Whakaana’s oxygen saturation (Elliot, Aitken Chaboyer, 2007). Pain and discomfort are also important factors in Mr Whakaana’s postoperative period as good pain control is required for an optimal physical and psychological recovery. Post-operative nausea and vomiting (PONV) is common after laparoscopic cholecystectomy because of peritoneal gas insufflation and manipulation of the bowel (Graham, 2008). There are additional risk factors to consider including the use of peri-operative opioids (REFERENCE). Opioids, such as morphine, are a common cause of PONV and so their use, even during laparoscopic cholecystectomy, should be kept to the required minimum. Pain should be measured using an assessment tool that identifies the quantity and quality experienced of Mr Whakaana’s pain. Patients’ self-reporting of their pain is regarded as the gold standard of pain assessment measurement as it provides the most valid measurement of pain (REFERENCE). Self-reporting can be influenced by numerous factors including mood, sleep disturbances and medications and may result in patients not reporting pain accurately (REFERENCE). For example, Mr Whakaana may not report his pain because of the effects of sedation or lethargy and reduced motivation as a consequence of the surgery. Fluid Balance / Output Patients following surgery are vulnerable to fluid and electrolyte imbalance due to many factors, including blood loss, fasting for long periods and exposure during surgery (Walker,2003). Therefore an accurate measurement of Mr Whakaana’s fluid balance is an essential factor in evaluating his condition. This should include strict readings of the output of drains as well as urine and vomit, and the measurement of fluid intake (oral, nasogastric and intravenous). Wound drainage sites and the surgical wound itself should be inspected at regular intervals for excessive blood loss, as this may indicate haemorrhage. Other factors that should be taken into account include diarrhoea, sweating and the use of diuretic therapy. Blood Sugars Diabetes is associated with an increased requirement for surgical procedures and increased postoperative morbidity and mortality (Dagogo-Jack Alberti, 2002). Hyperglycaemia impairs leukocyte function and wound healing (Tiziani, 2010). The management goal for Mr Whakaana is to optimize metabolic control through close monitoring, adequate fluid and caloric repletion, and sensible use of insulin (Dagogo-Jack Alberti, 2002). This assessment is to prevent hyperglycaemia and prevent further complications during Mr Whakaana hospital stay. Infection /Sepsis Conclusion Although postoperative care is a daily occurrence within many areas of practice, it is evident that the theory underpinning nursing actions is often forgotten in daily practice and hence actions may not be prioritised as they should be. It is hoped that this paper has enabled the reader to revisit the principles underpinning postoperative care. Such care must be viewed as a priority, and although there are local policies in place to guide nursing staff, the responsibility for understanding the reasons for actions lies with each individual practitioner. To explain the situation, I traveled to Manchester, England on January 22, 2005 through February 8, 2005. On January 25, 2005, my wife called to inquire about international calling plans offered by Verizon. She was informed by a Verizon employee that the calling plan cost $4/month and that would entitle her to $.08/minute to England. Based on this fact, we used the calling plan for the rest of the time I was England, assuming we were paying $.08/minute. However, when we received the phone bill, the charges for all calls made through the calling plan were charged at $.30/minute. When I contacted Verizon regarding the difference, I was told that each call had a premium service termination charge attached to it of $.22/minute. My problem with this situation is that my wife was never informed that a premium service termination charge existed. When she signed up for the plan, a Verizon employee specifically told her that all calls to England would be $.08/minute. She was not given a qualifier stating that there was a possibility the charges may be higher due to a premium service termination. Furthermore, we never received literature explaining the details of the plan. Had we known that charges could have been $.30/minute, we would have used an alternative calling plan with much lower rates. Due to false information, or a lack of important information, given to us by a Verizon employee, we were overcharged $89.

Civil Rights Movement in the United States, was a political, legal, and social struggle to gain full citizenship rights for African Americans and to achieve racial equality. The civil rights movement was a challenge to segregation, the system of laws and customs separating blacks and whites. During the civil rights movement, individuals and organizations challenged segregation and discrimination with a variety of activities, including protest marches, boycotts, and refusal to abide by segregation laws. Some believe that the movement began with the Montgomery bus boycott in 1955 and ended with the Voting Rights Act of 1965, there is still however some debate about when it began and whether it has ended yet. The civil rights movement has also been called the Black Freedom Movement, the Negro Revolution, and the Second Reconstruction. Segregation was an attempt by white Southerners to separate the races in every sphere of life and to achieve supremacy over blacks. Segregation was often called the Jim Crow system. Segregation became common in Southern states following the end of Reconstruction in 1877. By 1877 the Democratic Party had gained control of government in the Southern states, and these Southern Democrats wanted to reverse black advances made during Reconstruction. To that end, they began to pass local and state laws that specified certain places ?For Whites Only? and others for ?Colored.? Blacks had separate schools, transportation, restaurants, and parks, many of which were poorly funded and inferior to those of whites. Over 75 years, Jim Crow signs went up to separate the races in every possible place. The system of segregation also included the denial of voting rights, known as disfranchisement. Between 1890 and 1910 all Southern states passed laws imposing requirements for voting that were used to prevent blacks from voting, These requirements included: the ability to read and write, which disqualified the many blacks who had not had access to education; property ownership, something few blacks were able to acquire; and paying a poll tax, which was too great a burden on most Southern blacks, who were very poor. Because blacks could not vote, they were virtually powerless to prevent whites from segregating all aspects of Southern life.
